POTENTIAL PRACTICE MERGER

Dear Patients

 

Re. Merger for Dr Yerra and Dr Chakrabarti practice

We are taking the opportunity to write to you personally to inform you about important changes to your GP Surgery. 

I am sure that you are aware that over the past few years there have been increased pressures placed upon General Practice services. Our two practices, Dr Yerra’s Practice and Dr Chakrabarti’s Practice share the same commitment to quality and while both practices have had good recent CQC ratings, we have jointly decided that the best way to continue to offer our patients a quality efficient service is to merge our practices.

By merging we will be able to share working arrangements and work together to preserve high quality general practice for this area. We have begun to make arrangements for a merger from December 2018.

All of our doctors and our practice nurses and other staff will continue to be available to you under the new name of Dr Yerra Practice.

Your registration will not be affected and you will be able to be seen by one of our doctors or nurses at the same premises.

We are proud of both of our practices and the personal care we have maintained over the years and have always been touched by the positive feedback and support from our patients. We are pleased that our teams will be uniting as one like-minded team and will continue to provide care for you at our practices following a seamless and safe sharing of healthcare information and prescribing data.

We understand that you may have questions about the merger and we intend to address your concerns

We will be holding a Patient information evening. This will be an opportunity for you to ask some questions and meet some of the doctors and staff.

This will be a time of change for all of us at Dr Yerra’s and Dr Chakrabartis Practice and whilst we will all work extremely hard to minimise the disruption, we are grateful for your understanding during this time.

The surgery is offering Meningitis ACW & Y vaccinaiton to all 18 years old and up to 25 years if you are going to university.
