Register with our practice
The practice welcome new patients. We operate a policy of non-discrimination. Patients from within the practice area wishing to join the practice will need to complete a registration form, this can be collected from the reception or downloaded from link below.
Please complete the form and bring your medical card and if possible, a letter with current address. we would like you to be present when registering please try come at midday time so any query will be resolved quickly.
Please Note: After registration we offer a new patient health check appointment with Trainee Health care assistant or practice nurse.

Named GP
An accountable GP will be assigned to every patient and where a preference is expressed reasonable efforts will be made to accommodate this.
